What they offer

Supports local general practices and provides programmes that help older people manage their health and remain independent. Older People's Health services can connect seniors and whānau with appropriate health and community support, alongside wellbeing, palliative care and self-management programmes.

Cost

PHO enrolment is free. GP consultation and other practice fees vary between participating medical centres

Office Monday–Friday, 8.30am–5pm. Services and programmes vary; GP or practice nurse referral may be required
